Certified blind by a medical officer of the Eye Clinic or registered Ophthalmologist in private practice
In need of partial personal care and attention /
and nursing care
Without a family or family member(s) cannot take care the applicant
Mentally suitable for group living
Self-financed placement

ADMISSION / WITHDRAWAL PROCEDURES
Subvented placement
Applicants can apply from the Social Welfare Department, volunteer family service centers, or medical social services units. The referral will sent to Central Referral System of the Social Welfare Department for consideration.
After home visit, if the applicant is verified to be eligible for admission, Ebenezer Care and Attention Home will arrange for admission as soon as possible
If a resident would like to move out because of
adaptation and other personal reasons, one month’s notice in advanced is required.
Self-financed placement
Applicants can download or get application form from Ebenezer Care and Attention Home. Return the completed form and fax to 2817 4872 or mail to Ebenezer Care and Attention Home at 1/F 131 Pokfulam Road, Hong Kong. We will arrange
appointments and assessments afterwards.One month’s notice to Ebenezer Care and Attention Home one month before moving out.

Basic fee starts from $8000.00, depending on room type and level of care required.
